Iago also gets Roderigo to help him in his evil plan by telling him that he can have Desdemona if he helps get rid of Cassio. Oblivious to Iago’s master plan Othello is ultimately a victim of his own naivete. It all starts when Iago plants a little seed of suspicion into Othello’s mind about Desdemona. Iago states, I speak not yet of proof. Look to your wife; observe her well with Cassio; Wear your eye thus, not jealous nor secure: I would not have your free and noble nature, out of self-bounty, be abused; look to’t: I know our country disposition well; In Venice they do let heaven see the pranks they dare not show their husbands; their best conscience is not to leave’t undone, but keep’t unknown. (III. iii. 196-204) For the rest of the play all of Othello’s suspicions about innocent Desdemona are built upon this bug Iago has planted in Othello’s ear. Eventually, Othello confronted Desdemona and Emilia (Desdemona’s companion) about the situation Iago informed him of but it was already too late because his mind was already made up. However, he genuinely believe his ensign would never do such a thing, as he stated, â€Å"I think thou dost; and, for I know thou’rt full of love and honesty† (III. iii. 117-118). Instead of Iago just coming to Othello and saying, â€Å"Cassio is sleeping with Desdemona, † he made it more believable by pretending he really didn’t want to tell Othello of his suspicions and acted as if he was trying to protect Othello from jealousy. Of course, Iago’s actions only increased Othello’s desire to know what Iago thought which worked perfectly for Iago’s plan. Iago knows very well that trust and deceit go together in order for him to complete his master plan. So the whole time he plans Othello’s downfall he is continually trying to get his undoubting trust. Slowly but surely he poisons people’s thoughts, creating ideas in their heads without implicating himself. Iago even says himself that the advice he gives is free and honest which makes people believe him even more! Another one of the main people Iago deceives is Roderigo. Throughout the play, Iago tells him that he hates Othello and that Roderigo should make some money so he could give gifts to Desdemona, who Roderigo admires from afar. Without even a second thought, Roderigo sells his land and uses all his savings to buy gifts for Desdemona. However, Iago is actually keeping the gifts that Roderigo plans to give Desdemona for himself. Eventually, Roderigo begins to catch on to the act and confronts Iago, but he falls right into Iago’s trap again when he tells him that killing Cassio will help him win over Desdemona because Desdemona is falling more in love with Cassio. Roderigo is then led to his death by the hands of â€Å"Honest Iago† when he draws his sword hoping to kill Roderigo while Roderigo is fighting Cassio. The next incident which furthers Iago’s plan is when Othello sees Cassio walking away very quickly after conversing with Desdemona. To make sure Othello does not get suspicious, Iago whispers, â€Å"†¦I cannot think it, that he would steal away so guilty like. † (lll,iii,42) Othello then replies, â€Å"†¦I do believe ‘twas he. † (lll,iii,44) Othello believes what he sees to be the truth even though he didn’t hear what was said. This instance is only the beginning of the lies that he sees from afar. However, Iago does not stop there because he cannot predict every time the two will meet. Therefore, he decides to manipulate Othello’s feelings for Desdemona again. There are a few people who know of the handkerchief Othello gave to Desdemona when they began courting. In fact, Cassio was there when she received it and Desdemona promised to never leave it out of her sight. But, that all changed when Othello yelled at her the first time and she dropped it. Emilia, (her companion and Iago’s wife) picked it up and brought it to her husband which only furthered Iago’s horrible scheme and little does she know, â€Å"Emilia is just another pawn in his game† (Polymath). â€Å"Iago’s deception depends on the image of the handkerchief in Cassio’s hand, and he achieves this image not only through description, but by directing the actions of Cassio and Bianca by showing that the handkerchief is no longer in Desdemona’s possession† (Smith 33). Once Iago had the handkerchief in his possession he planted it in Cassio’s home hoping Othello would find it or Cassio would use it in front of Othello. Iago tells Othello he will start a conversation with Cassio about Desdemona and all the proof he will need shall be there. Iago places Othello where he can see but cannot hear. Iago then starts a conversation with Cassio about Bianca. However, it appears to Othello that Cassio is talking in a sexual way about Desdemona. In reality, Iago places Cassio in a position to talk about how he really feels about Bianca. Othello sees this from afar and says in shock, â€Å"Crying, ‘O dear Cassio,’ as it were, his gesture imports it. (lll,i,155) Othello believes the appearance of the conversation, which for him is all the evidence he needs. Now, since Othello trusts Iago, he believes everything Iago says to him and trusts his opinions, even though they are in reality, lies. Iago supports everything he has led Othello to believe and never wavers from his thoughts or offers the benefit of the doubt because he doesn’t want to make Othello doubt him especially after he gained his trust. â€Å"Because he wants material proof, he equates love with a handkerchief which in its very aterial certainty is most subject to chance. Thus the plot ironically emphasizes that his desire for certainty renders him most subject to chance, operating on the most trivial levels, as well as to Iago’s malevolent manipulation of it† (Stockholder 256). In the end, Othello kills Desdemona because he couldn’t take the thought of her being with another man and lying to him when she was really truthful. Then Emilia tells Othello the truth behind the handkerchief and how her husband, Iago is evil before she is killed by Iago. Roderigo also dies from his wounds inflicted while fighting Cassio and unknowingly fighting Iago. Othello then kills himself when the realities of all those appearances are brought to the light. Appearance versus reality is a recurring theme throughout the play. By jumping to conclusions and not getting both sides of the story Othello and Roderigo fall to their own demise. These later stelae are carved from andesite, and the persons depicted have impassive faces and wear elaborately woven tunics, sashes, and headgear of elites. The people in these carvings have three-dimensional shoulders, head, arms, legs, and feet. They often hold equipment associated with the use of hallucinogens: a kero vase full of fermented chicha and a snuff tablet for hallucinogenic resins. Overview of Sarcopenia Background Sarcopenia is age related disease with symptoms of loss of muscle mass, strength and function. Elderly over the age of 65 years are vulnerable to sarcopenia [6]. It is estimated that approximated 5-13% of older people aged 60–70 years are suffered from sarcopenia [8]. The proportion is about twice higher among elderly at the age of 80 or above [8]. Studies also found that sarcopenia is more likely seen in older men than older women [16,26]. Sarcopenia gains intensive attention from public and increasing researches indicated that it is a major clinical problem for older people. Risk factors of sarcopenia Current research found that lifestyle factors, including physical inactivity, smoking and alcohol consuming [3]; and biological factors, including older age, gender, decreased hormone level, motor unit remodeling and reduced protein synthesis [3,25], contribute to development of sarcopenia. Motor unit remodeling comes up of age and leads to replacement of fast twitch motor neuron[22,25] which results in less precise control of movements, less force production and slowing of muscle mechanics[22,23,25] as remodeled motor unit are smaller in size and slower to contract. Therefore, loss of fast twitch fibers increases risk of having sarcopenia. In addition, protein synthesis, growth hormone (GH), testosterone (T) and insulin-like growth factor (IGF-1) are considered to be associated with sarcopenia as well [25]. It is well known that protein is important in muscle repairmen. And GH, T and IGF-1 are involved in protein metabolism and maintenance [23]. Different studies agreed that protein synthesis rate decreases throughout the natural aging process [24] and leads to loss of muscle mass. Thus, low protein synthesis rate, along with decrease in these hormones level provide possible occurrence of sarcopenia [25]. Consequences of sarcopenia Sarcopenia causes serious consequences not only at individual level but also at societal level. On the one hand, loss of muscle mass, strength and function lead to adverse health outcome in terms of frailty, disability, morbidity and mortality [8]. Essentially, sarcopenia is about twice as common as frailty [9]. Also, sarcopenia occurs with other morbidity in some times. Some of the co-morbidity are obesity [4,13], hypertension, osteoporosis [12] and type II diabetes [5,14,15]. In United States, the estimated direct healthcare attributable to sarcopenia represented about 1.5% of total healthcare expenditure in 2000 [21]. Diagnosis of sarcopenia according to different consensus panels Although research working in the area of sarcopenia is expanding exponentially, a universal definition is still under development. On average, current definitions are including muscle mass, muscle strength or even physical function. In 2010, the European Working Group on Sarcopenia in Older People (EWGSOP) published guidelines to help identify sarcopenia [1,9]. According to the EWGSOP, a person will be classified as having sarcopenia when two of three follow criteria were statisfied: (A) low muscle mass and (B) low muscle strength and/or (C) low physical performance [1]. Low muscle mass is defined as muscle mass ≠¥2 standard deviations below the mean of reference population [8], calculating by equipment such as DEXA-scanners. Low physical performance is defined as gait speed ≠¤ 0.8 m/s in the 4 meters walk test for both males and females [8]. International Working Group on Sarcopenia (IWGS) suggested diagnosing sarcopenia when the following criteria are fulfilled: (a) gait speed was < 1 m/s and (b) low muscle mass (cutoff value is similar to EWGSOP) [8]. The European Society of Parenteral and Enteral Nutrition Special Interest Groups carried out that the cut off values for low muscle mass is defined as percentage of muscle mass > 2 SDs below the mean of reference individuals; for walking speed is < 0.8 m/s in the 4 meters walk test [1]. Measurement of muscle mass and muscle strength Muscle strength is mainly assessed by handgrip. While physical performance can be measured using simple tests such as the short physical performance battery test, usual gait speed or the timed get-up-and-go test; it is difficult to assess muscle mass in practice. Dual-energy X-ray absorptiometry (DEXA) is recently proposed as the gold standard for muscle mass measurement. Other methods include bioelectrical impedance, computed tomography, magnetic resonance imaging, urinary excretion of creatinine, anthropometric assessments, and neutron activation assessments can be used for measurement of muscle mass as well [8]. The process is complicated and need participation of professionals. Prevalence of sarcopenia varies as use of cut-off points relies on different instruments used for assessing muscle mass and strength and function. SARC-F, a newly developed simple questionnaire, has been regarded as rapid diagnosis test for sarcopenia. It contains five components: strength, assistance in walking, rise from a chair, climb stairs and falls. A question will be asked to assess each component variable. Scores range from 0-10, with 0-2 for each component [7]. Details of SARC-F are shown in table 1. Cut-off score A research conducted in Hong Kong tests the validation of SARC-F as a screening tool for sarcopenia in community. It found that SARC-F is able to predict future adverse outcomes with comparable power to other criteria. It also found that SARC-F has excellent specificity (94.4%) and negative predictive value but poor sensitivity. With high specificity, SARC-F is useful for screening out older adults with sarcopenia. Poor sensitivity may due to the number of participants classified as having sarcopenia represent only a small proportion of the total population studied [10]. Another research applied SARC-F in mainland China to screen sarcopenia and physical disability. It published that poor physical performance and grip strength were associated with SARC-F defined sarcopenia. But there was a very weak correlation to muscle mass in physically active outpatients. In general industrial and system engineering tools can help the ambulatory surgery centers in different way, decrease the cost by decreasing the length of stay for patient: because most of the cases in the day surgery unit is normal and repeatable surgery (most of them are easy not complicated surgery) then the procedure can be standardized, this standardization will reduce the cost by removing the unnecessary process from the units. It is know that the nature of hospital is not same and there is high level of variation between different cases, this problem can be reduced in th e ambulatory surgery centers. Any engineering and system tools or techniques can be very useful in ambulatory surgery centers, in mean of the capabilities of providing the patient with excellent treatment within short period of time, both will result to patient satisfaction and improving the patient recovery time after leaving the ambulatory surgery centers, and also reduce the cost on ambulatory surgery center by reducing the number of employee and resources needed, because the good engineering tools mean less waste and more efficiently for both employee and resources. It can be summarized into following points: Decreased salaries expense by reducing the staffing requirements. Greater patient satisfaction through reduced waiting times. Increases employee satisfaction because workload is evenly distributed and activity levels do not create a hurry up and wait environment.
